• UHS employees and their covered dependents who are enrolled in an Aetna, Blue Cross of TN, or UnitedHealthcare health plan.
• You (and/or your covered dependents) must have a Type 1 or Type 2 Diabetes diagnosis to enroll.
• Your enrollment information is not shared with your employer.
